Provide one of these documents as proof of immigration status:
Birth Certificate
You can get a copy of your birth certificate through the Wisconsin Vital Records Office. To obtain a birth certificate you must have valid photo ID. If no photo ID is available than two separate forms of proof of ID are required. The application is available to download online but you can only apply in person, by mail or by fax.
Passport
If this is your first time applying for a passport, you must apply in person. You can apply at various government locations including courthouses and the post office. You will need a valid form of proof of ID, proof of US citizenship (link to citizenship tab) and two photographs of yourself. These photos must be 2" by 2". Visit your local photo shop to have your passport photos printed in this exact size.
Naturalization Certificate
You can get a copy of your certificate by filing form N-565 (available online) with the USCIS either in person or by mail. You will need to provide two recent, identical photographs and the old certificates if you have it. The instructions for size and background of the photos are available on the application.
Lawful Permanent Resident Card (Green Card)
If you need to replace or renew your Green Card, you can do it
online or by mail by filling out form I-90.
USCIS (INS) Immigration Forms

- USCIS I-210 letter (Voluntary Departure)

- Letter from USCIS (INS) that shows that you are living in the U.S. with the knowledge and permission of the USCIS (PRUCOL status)
Baptismal Certificate
To obtain a copy of your baptismal certificate, contact the church where the ceremony was performed. This document should be an early public record, preferably created within the first five years of life.
