income if it applies to your household:
Child and adult dependent care expenses
- Canceled checks or receipts from childcare provider
- Letter from provider that says how much you paid and how often they provided care
Heating, telephone and electricity costs
- Bills or statements from utility companies
Rent or mortgage payments
- Canceled checks or receipts for rent paid
- Mortgage statement
Medical expenses (only medical expenses not covered by an insurance program)
- Bill or invoice from doctor, hospital or clinic
